BP-II patients were recruited from outpatient and inpatient settings. All were initially evaluated in an interview by an attending psychiatrist and followed by a more detailed interview with a clinical psychologist using the Chinese Version of Modified Schedule of Affective Disorder and Schizophrenia-Life Time (SADS-L) [59], which has good inter-rater reliability [60], to determine Diagnostic and Statistical Manual of Mental Disorders, fourth edition (DSM-IV) diagnoses. Inclusion criteria were a diagnosis of BP-II, either first-onset or with previous episodes. Exclusion criteria were (i) any DSM-IV-TR Axis I diagnosis including substance use disorder, organic mental disorders, and major and minor mental illnesses except BP-II and lifetime AD, (ii) any significant medical illness, (iii) any neurological disorders, and(iv) any poorly controlled physical illness that might influence the interview and study results. The diagnosis of AD was based on DSM-IV-TR criteria. Although DSM-IV-TR [3] criteria require a minimum duration of 4 days of hypomania, current epidemiologic data suggest that a 2-day duration is more prevalent in community samples [21,61]; therefore, we used the 2-day minimum for hypomania in the diagnosis of BP-II. The healthy control group (here after Controls) consisted of 340 volunteers recruited from the community. We used the SADS-L [59] to screen their psychiatric conditions. All volunteers were free of major and minor mental illness (affective disorder, schizophrenia, anxiety disorder, personality disorder, substance use disorders, etc.), and none had a family history of psychiatric disorder among their first-degree relatives. |

Logistic regression analysis showed a significant effect of the COMT and the BDNF polymorphisms, and a significant interaction effect for the Val/Val genotypes of the BDNF Val66Met polymorphism and the COMsT Val158Met Val/Met and Met/Met genotypes (P=0.007, 0.048) discriminated between BP-II without AD patients and controls. Our findings provide initial evidence that the COMT and BDNF genes interact in bipolar-II without AD. Our findings suggest the involvement of dopaminergic pathway in the pathogenesis of bipolar-II. |
